Please hand these completed forms to reception when you arrive. You may also fill these forms at our centers but please arrive an extra fifteen minutes prior to your exam to complete them.

Note – for the assignment of benefits/release of information form, please enter the date of your scan (date of service) when signing this form.

For patients having a breast MRI, please download the following form and hand in to reception when you arrive.

Insurance
Partners is contracted with the majority of insurance carriers including Medicare.

If you have any questions regarding your insurance coverage, simply call us at (941) 951-2100 or (941) 782-0414 and request that your benefits are checked for you.  One of our staff members would be pleased to call your insurance carrier and check this for you.
